The Effects of Perioperative Ketorolac Infusion on Platelet Function and Blood Loss

Abstract
Ketorolac is a potent nonsteroidal antiinflammatory drug available for parenteral use with analgesic and opioid-sparing qualities that are well documented. Despite this, there is concern over the use of ketorolac in the perioperative period that it might induce excess surgical site bleeding. The purposes of this study were to investigate the effects of perioperative continuous infusion of ketorolac on platelet function and blood loss. The study comprised 40 patients undergoing general anesthesia for total knee arthroplasty. The patients received ketorolac 30 mg(K group, n £½ 20) or saline 1 ml(C group. n £½ 20) intravenously 10 min before skin incision. followed by a continuous infusion of ketorolac (2.5 mg£¯h) or saline(4 ml£¯h) for 24 h. Blood loss was collected and measured at 3, 6, 12 and 24 h after skin incision. Platelet count, hemoglobin, platelet aggregometry(PA) with adenosine diphosphate(ADP) and collagen, bleeding time(BT) were measured at 10 min before skin incision and 3, 24 h after skin incision. Blood coagulation status was analysed using thromboelastography(TEG) at 10 min before skin incision and 3 h after skin incision. BT decreased significantly in the C group from preoperative value to postincision but BT in the K group did not change. PA with ADP did not change significantly but PA with collagen after postincision were lower in the K group than in the C group. The TEG data was unchanged in both groups. No significant differences were observed operative blood loss.
